New Patient Information

To join the surgery, you usually have to live within our practice boundary, you can check on the boundary map below.

The registration process takes around 15 minutes to complete. The form can be found here and on the ‘Join the Practice’ tile on our home page.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

If you are unable to complete the form online you can collect a paper copy from reception or download and print the registration form

Registering a baby or child

Children under 16 usually need to be registered by their parent or guardian. This means filling in a separate form.

We cannot register a child under 16 without their parent or guardian being registered here too.  When registering a baby or child you will need to –

• be registered with the same GP surgery as the child
• need to provide proof of ID and address for the child you’re registering, such as their birth certificate
• provide ID by the surgery to prove you’re the parent or guardian of the child
• Provide the personal child health record (red book), if you have it, or copies of their vaccination records.

Practice Boundary Areas

The Green Surgery

The Practice catchment area runs along King St past Twickenham Station, down Whitton Rd, along the A316 to Apex Corner. It then comes back along the A305 and turns right into Sixth Cross Road and then turns down Wellington Road to Hampton Road. It then runs along Hampton Road and heads north up A309, Waldegrave Road joining the A310 back to King Street.

Fir Road Surgery

The Practice catchment area begins at Nallhead Rd at its junction with the A316 and then continues between the two reservoirs turning south along the railway line until the junction with Uxbridge Rd. It then turns north on the A312 until the River Longford runs adjacent at which point the line bears left and follows the river until it runs under the A316 at Apex Corner. The line turns left and follows the A316 until the junction with Nallhead Road
