Access to Your Medical Records

If you’d like to view or receive a copy of your health records, you can make a Subject Access Request (SAR). You can submit this yourself or someone you’ve authorised to act on your behalf can do this, in line with the GDPR and Data Protection Act 2018.

To submit a request, please fill out our Subject Access Request Form – available online here SAR Request Form or in paper format. If you need the paper version, this can be collected from reception. Once you have completed the form please return this to reception in person.

Once your request is received, we’ll get in touch to arrange a time for you to collect or view your records.

• There is no charge for this service.

• You will need to show proof of identity before accessing your records. E.g Passport or Driving License

How long will it take?

• We aim to respond to your request within 30 days.

• If we need more information from you to complete the request, the 30-day period will start once we receive that information.

• For complex or multiple requests, the timeframe may be extended by up to two additional months. If that happens, we’ll let you know within the first 30 days and explain why the extension is needed.
